1.South Florida School of Floral Design agrees to
enroll the above student in the following program.

TITLE OF PROGRAM: FLORAL DESIGN AND SHOP MANAGEMENT TOTAL CLOCK
HOURS AND WEEKS: 105 CLOCK HOURS- 3 WEEKS TOTAL TUITION:
$1,250.00
Registration fees: $50.00. Tuition Balance. $1,200.00,
Payable on first morning of class.
Tuition: $1,250.00 for the complete 3 week program
includes all fees, quantities of flowers and foliages, working
materials and other floral materials used in the classroom.
Weekly tuition is $450.00 per week, if phases are taken
separately. (The total three week program, taken without
interruption is only $1,250.00)
2.Rules and Regulations:
The school has the right to dismiss/discontinue a
student for failure to maintain satisfactory academic progress,
failure to attend classes punctually , breach of school rules or
regulations, or for any cause the school deems necessary for the
good of the school. The transcript is to be withheld until all
fees and charges have been met.
3.Cancellation & Refund Clause:
A student may cancel this enrollment agreement and all
monies will be refunded by notifying the school in writing
within three business days after midnight of the day on which
this agreement was signed. After the three day cancellation
period, if a student fails to enter, the charge will be the
registration fee only. If a student withdraws or is discontinued
before completion of the program, the sum charged as of the last
day of attendance will be:
A. A student withdrawing or being terminated after entering
SCHOOL and starts training, but before 50% of the program is
completed, shall receive a prorated refund. This refund is based
upon the number of clock hours completed to the number of total
program clock hours and amount of monies paid.
B. No refunds are made after 50% of program is completed.
C. Refunds are made within 30 days of withdrawal.
D. All withdrawal request must be in writing and dated.
E. For the purpose of computing refunds, the last day of actual
attendance of student will be considered termination date unless
prior written and dated notice received.
F. Notice of cancellation or withdrawal must be made by
Certified mail.
4.Contract Rejection
In the event that the school rejects this contract, all
payments made by me in compliance with this contract are to be
refunded in full.
TRUTH IN LENDING STATEMENT: All prices for
programs are as printed herein. There are no carrying charges,
interest charges, or service charges connected or charged with
any of these programs. Contracts are not sold to a third party
at any time. Cost of credit is included in the price cost for
the goods and services.
EMPLOYMENT: The school offers to its graduates
a placement service. Placement service does not constitute a
guarantee of employment.
DIPLOMA: Upon successful completion of program,
the STUDENT will be awarded a diploma.
STUDENT AGREEMENT: This agreement constitutes a
binding contract upon acceptance by the SCHOOL except as noted
in the cancellation section.
NOTICE TO BUYER: DO NOT SIGN THIS CONTRACT BEFORE YOU
READ IT OR IF IT CONTAINS ANY BLANK SPACES. YOU ARE ENTITLED TO
AN EXACT COPY OF THE CONTRACT YOU SIGNED. KEEP IT TO PROTECT
YOUR LEGAL RIGHTS. ALL SIGNERS HAVE RECEIVED AND READ A COPY OF
THE BINDING DOCUMENT AND CATALOG.
